Internal hemorrhoids often are small, swollen veins in the wall of the anal canal. But they can be large, sagging veins that bulge out of the anus all the time. They can be painful if they bulge out and are squeezed by the anal muscles. You may experience itching, burning, bleeding and pain, before, during and after bowel movements.
External hemorrhoids can clot or thrombose causing severe anal pain prompting and emergent doctors office visit for drainage. When this happens it feels like an egg between your buttocks and is very painful to the touch. Symptomatic hemorrhoids develop as a result of heavy lifting, diarrhea, straining, or prolonged sitting time on the commode. Once they are bulging from the anus the disease typically progresses and symptoms will worsen. Creams and other topical agents will not change the progression of disease and give only temporary relief.
